protect the data you provide during your visit to our facility or through our communication

   We collect and store personal and medical information provided voluntarily by clients in our
   intake forms, including:
   ● Full name, contact information (phone number, email)
   ● Medical history relevant to procedures
   ● Record of procedures performed
   ● Photos taken before and after treatments (with consent)
2. Use of Information
   The collected information is used strictly for the following purposes:
   ● To create a customized treatment plan
   ● To document procedure progress
   ● To communicate with you about your appointments or follow-ups
   ● For internal record-keeping and treatment history tracking
3. Data Confidentiality
   All personal data is stored securely and accessed only by authorized personnel. We do not
   share, sell, or rent your information to third parties. Your medical and contact information will
   never be disclosed without your written consent, except where required by law.
4. Photo Consent
   With your signed approval, we may take and store photographs of treatment areas for
   progress tracking. These images are used internally only, unless you provide separate
   written consent for promotional use.
5. Cancellation & Refund Policy
   ● We kindly ask for at least 24 hours’ notice for appointment changes or
   cancellations.
   ● Cancellations within 24 hours will incur a $25 fee.
   ● Missed standing appointments (3 consecutive no-shows) require a deposit for
   rescheduling.
   ● No refunds for services rendered or opened products. Unused services may be
   exchanged for other services offered at our facility.
6. Your Rights
   You have the right to:
   ● Access or request a copy of the information we hold about you
   ● Request correction of inaccurate or outdated data
   ● Withdraw consent for photo use at any time

   This policy may be updated periodically. The latest version will always be available upon
   request at our reception or on this page.
